All meetings reguire pre-registration either by phone call or e-mail. With few exceptions, registration is limited to 40 participants. If registrations exceed the number of open slots, you may ask that your name be put on a waiting list. All participants must sign a liability form. These forms are available at the registration/attendance table at each meeting. Only one form per year (2010-2011 Academic Year) is necessary. You may also download and fill out the form ahead of time, bringing it to the meeting with you.
Photos of our activities in hub meetings may be included on the web site. No participant will be named. If you do not wish to be included in the photos that will be posted on the web, please indicate this on the liability form, in comment section of your registration and mention it to the photographer at the hub meeting.
You will be able to sign up for Hub Meetings only one month at a time. You may sign up for the next month's Hub meeting on the day following the previous meeting, or, if you are present, you may sign up for next month at the previous month's meeting.

Please register individually rather than as a group.
When you RSVP please include the following information:

- Today's Date
- Meeting reservation date
- School District
- School
- Email Address
- Phone number for message/contact to confirm your registration
- Any comments or questions regarding the meeting.
In compliance with Colorado School of Mines regulations, children of participants should not attend Hub meetings or activities. Thank you for your cooperation.
The PALS Liability Form
All participants are required to complete and turn in to Tammy Johnnie a liability form. It is only necessary to fill out one form per academic year. Please download and print out this form. Bring it to the meeting and give it to Tammy Johnnie.
All participants must complete the LIABILITY FORM.
